Be sure to fill out the ORDER FORM and have it present before you call to place your order by telephone. Read all of the information from your completed ORDER FORM when EPOS (Electronic Phone Ordering System) indicates it is ready to take your order. Please spell out any names, streets, cities, etc. This ORDER FORM can be printed by clicking your mouse cursor on the print button on your browser's tool bar. When you want to order by fax you should fax a completed copy of the ORDER FORM or include all of the information requested on the ORDER FORM on your letter head or fax cover page. When ordering by email send your order to sales@10001press.com. Include all the information requested on the ORDER FORM in your email transmission.

Payment can be made by check, (including check by fax and email), money orders, banker's cashier checks, postal money orders, cash, VISA and Master card. Checks should be in U.S. Dollars only using a check form that bears a machine-readable magnetic sorting number drawn on a U.S. Bank. Credit card transactions are automatically converted to U.S. Dollars without any service charge to you. Non-negotiable checks or unauthorized credit card charge-backs are subject to a $25.00 accounting fee, plus collection cost.
